Filling out this brief questionnaire will help us to gauge interest in upcoming online classes and place your name on a priority list for the selected class. Indicating a preferred class size will help us place your student in a class that meets your financial needs. Please see our FAQ and our fee schedule for more information.

Please fill out this form for each different type of class you are interested in. For example, if you want to sign 3 children up for one class, you only need to fill out this form once. If you are interested in signing 2 children up for one class and 1 for another, you would submit this form twice.
